SMART-Modell
Das SMART-Modell hilft uns, Pflegeziele richtig zu formulieren.
S – Spezifisch: Das Ziel ist eindeutig und konkret.
M – Messbar: Die Zielerreichung ist überprüfbar.
A – Attraktiv / Akzeptiert: Das Ziel ist sinnvoll und wird vom Patienten akzeptiert.
R – Realistisch: Das Ziel ist erreichbar.
T – Terminiert: Es wird ein Zeitraum festgelegt.
ZWERG-Modell
Das ZWERG-Modell hilft uns, die Qualität eines Indikators zu überprüfen.
Z – Zentral: Der Indikator bezieht sich direkt auf das Ziel.
W – Wirtschaftlich: Die Messung ist mit angemessenem Aufwand möglich.
E – Einfach: Der Indikator ist leicht verständlich und anwendbar.
R – Rechtzeitig: Die Ergebnisse liegen rechtzeitig vor.
G – Genau: Die Messung ist präzise und zuverlässig.
Indikator
Der Indikator ermöglicht uns, die Entwicklung des Pflegeziels zu überprüfen.
Evaluation
Die Evaluation zeigt uns, ob das Pflegeziel erreicht wurde.
Die Evaluation kann in folgenden Intervallen durchgeführt werden.
Anlassbezogen: z.b Bei Veränderungen des Gesundheitszustands.
Regelmäßig: In individuell festgelegten Zeitabständen.
Bei Bedarf: Wenn die bisherige Maßnahmenplanung nicht mehr zur aktuellen Situation des Patienten passt.
